Application for Funding
Document | Description |

Application for Funding Online form necessary to complete the Strengthening Career and Technical Education for the 21st Century (Perkins V) Application for Funding, including the Budget/Expenditure Schedule. |

Intent to Apply Form Form is required for all new or returning (after an absence) LEAs wanting to apply for Perkins funding. Form must be submitted by January 31st to apply for funds beginning the following July. |

Local Educational Agency Amendment Form is required for LEA changes from the previous year's application that may affect the LEA's grant award notification such as an official LEA name change, consortium changes (members joining or leaving, disbandment), or if an LEA would like to opt out of Perkins. This form must be submitted as soon as the changes are known by the LEA. |

Enrollment Report to Establish Eligibility for Section 132 Postsecondary Funds Form to report an unduplicated count of adults (eligible recipients of Section 132 funds) meeting the economically disadvantaged career technical education enrollment criteria. |

Fall Term Enrollments in ROCPs and Adult Schools Form needed to complete the annual report of fall enrollments in Regional Occupational Centers and Programs (ROCPs) and adult schools. These data are required for the division of the Perkins V, Title 1, Part C funds between secondary and postsecondary programs. |

CDE 21 Course List Guide Guide provides a list of courses that should not be counted on the CDE 21 form or assisted with Perkins funds. |

Memorandum of Understanding Section 131 The Memorandum of Understanding (MOU) form is required for any Section 131 consortiums receiving Perkins funding. It is the responsibility of Consortium Lead to submit an updated form reflecting any and all changes that may occur. |
